dc.description.abstractAs fístulas oroantrais são comunicações epitelizadas que permitem o acesso direto entre a cavidade oral e o seio maxilar, resultantes de comunicações oroantrais não tratadas. Essas condições são comumente associadas a extrações dentárias em região posterior de maxila, geralmente por acidentes transoperatórios ou devido à relação entre as raízes dentárias e o seio maxilar. O tratamento cirúrgico é indicado quando há defeitos ósseos > 3mm, sendo a utilização da bola de Bichat uma excelente alternativa para defeitos maiores. Objetivou-se relatar um caso de tratamento cirúrgico de extensa fístula oroantral crônica utilizando o coxim adiposo bucal. Paciente JMA, sexo feminino, 26 anos, compareceu à Clínica Escola de Odontologia da Unichristus com queixas de dor facial e passagem de líquido para o nariz, após exodontia realizada há 2 meses, outro serviço. Ao exame físico, observou-se presença de orifício na região do dente 26 circundado por mucosa oral normocrômica. A tomografia computadorizada evidenciou defeito ósseo e sinusopatia na região, sendo estabelecido o diagnóstico de comunicação oroantral. Foi prescrita antibioticoterapia prévia por 7 dias, sendo estendida após a abordagem cirúrgica juntamente com medicações analgésicas e anti- inflamatórias. A cirurgia consistiu na curetagem e limpeza do alvéolo, incisão relaxante e acesso à bola de Bichat, com posterior posicionamento para fechamento da comunicação. O acompanhamento de 7 dias evidenciou cicatrização satisfatória, sendo realizada laserterapia de baixa potência para fotobiomodulação e remoção de sutura no 14o dia. Conclui-se que o diagnóstico das fístulas e comunicações oroantrais, associado à indicação correta do tratamento, são essenciais e indispensáveis ao prognóstico e ao restabelecimento de parâmetros estéticos e funcionais do paciente.pt_BR

dc.description.resumo_abstractOroantral fistulas are epithelialized communications that allow direct access between the oral cavity and the maxillary sinus, resulting from untreated oroantral communications. These conditions are commonly associated with tooth extractions in the posterior region of the maxilla, usually due to intraoperative accidents or due to the relationship between the tooth roots and the maxillary sinus. Surgical treatment is indicated when there are bone defects > 3 mm, and the use of the Bichat ball is an excellent alternative for larger defects. The objective of this study was to report a case of surgical treatment of an extensive chronic oroantral fistula using the buccal fat pad. Patient JMA, a 26-year-old female, came to the Unichristus School of Dentistry Clinic with complaints of facial pain and passage of fluid to the nose, after an extraction performed 2 months ago at another service. On physical examination, the presence of an orifice in the region of tooth 26 surrounded by normochromic oral mucosa was observed. Computed tomography revealed a bone defect and sinus disease in the region, and a diagnosis of oroantral communication was established. Antibiotic therapy was prescribed for 7 days, which was extended after the surgical approach, together with analgesic and anti-inflammatory medications. The surgery consisted of curettage and cleaning of the alveolus, relaxing incision and access to the Bichat ball, with subsequent positioning to close the communication. The 7- day follow-up showed satisfactory healing, and low-level laser therapy for photobiomodulation and suture removal were performed on the 14th day. It is concluded that the diagnosis of oroantral fistulas and communications, associated with the correct indication of treatment, are essential and indispensable for the prognosis and reestablishment of the patient's aesthetic and functional parameters.pt_BR
