dc.description.abstractA Picnodisostose (PYCD) é uma condição genética rara de caráter autossômico recessivo, que apresenta uma prevalência de 1 em 1,7 milhão, causada por mutações no gene CTSK. Os principais achados orofaciais incluem displasias ósseas, suturas cranianas separadas, micrognatia, bossa frontal e occipital, hipercementoses, palato sulcado e hipoplasia mandibular. Devido ao metabolismo ósseo alterado, os pacientes ficam mais suscetíveis à osteomielite, podendo ser agravada por fraturas. Objetivou-se relatar e discutir sobre diagnóstico e manejo osteomielite mandibular na prática odontológica, a partir do relato de caso de tratamento cirúrgico de uma paciente com osteomielite mandibular atendida desde 2019 na Clínica Escola de Odontologia do Centro Universitário Christus. O exame clínico intraoral revelou palato sulcado e atrésico, além de alteração anatômica e presença de supuração no dente 34, porém, a paciente abandonou o tratamento. Em 2024, retornou com dor persistente no dente 34 e com bolsa periodontal de 15mm. A tomografia computadorizada constatou perda óssea acentuada em maxila e mandíbula, confirmando osteomielite na região do elemento 34. O tratamento consistiu na exodontia do dente 34, aplicação de terapia fotodinâmica antimicrobiana (aPDT) e cobertura antibiótica com Amoxicilina. Estudos destacam a osteomielite mandibular como uma das complicações frequentes em indivíduos com PYCD, sendo o desbridamento cirúrgico e a antibioticoterapia os principais tratamentos. Evidências recentes mostram que a aPDT é eficaz no auxílio do tratamento da infecção e na cicatrização. A combinação destas terapias citadas e do desbridamento cirúrgico resultou no sucesso pós-operatório. Conclui-se que a PYCD impacta diretamente no atendimento odontológico, sendo observadas dificuldades no planejamento do caso e possíveis intercorrências, exigindo do cirurgião-dentista conhecimento aprofundado desta condição, visando realizar um plano de tratamento eficaz e promover melhoria na qualidade de vida do paciente.pt_BR

dc.description.resumo_abstractThe Pycnodysostosis (PYCD) is a rare autosomal recessive genetic condition with a prevalence of 1 in 1.7 million, caused by mutations in the CTSK gene. The main orofacial findings include bone dysplasias, separated cranial sutures, micrognathia, frontal and occipital bossing, hypercementosis, grooved palate, and mandibular hypoplasia. Due to altered bone metabolism, patients are more susceptible to osteomyelitis, which can be aggravated by fractures. This study aimed to report and discuss the diagnosis and management of mandibular osteomyelitis in dental practice, based on a case report of the surgical treatment of a patient with mandibular osteomyelitis who has been treated since 2019 at the Dental School Clinic of the Christus University Center. Intraoral clinical examination revealed a grooved and atretic palate, as well as an anatomical alteration and the presence of suppuration in tooth 34, however, the patient abandoned the treatment. In 2024, he returned with persistent pain in tooth 34 and a 15 mm periodontal pocket. A computed tomography scan revealed significant bone loss in the maxilla and mandible, confirming osteomyelitis in the region of tooth 34. Treatment consisted of tooth extraction, antimicrobial photodynamic therapy (aPDT), and antibiotic coverage with amoxicillin. Studies highlight mandibular osteomyelitis as a common complication in individuals with PYCD, with surgical debridement and antibiotic therapy being the main treatments. Recent evidence shows that aPDT is effective in aiding infection treatment and healing. The combination of these therapies and surgical debridement resulted in postoperative success. It is concluded that PYCD directly impacts dental care, with difficulties observed in case planning and possible complications, requiring the dentist to have in-depth knowledge of this condition, aiming to carry out an effective treatment plan and promote an improvement in the patient's quality of life.pt_BR
