dc.description.abstractO reimplante intencional é uma alternativa terapêutica conservadora indicada em casos de falha do tratamento endodôntico convencional, possibilitando a manutenção do elemento dentário e evitando a exodontia. Dentes com anatomia radicular complexa, como molares com canal em forma de “C”, representam um desafio clínico devido às dificuldades de instrumentação, desinfecção e obturação adequada do sistema de canais radiculares. O presente trabalho tem como objetivo relatar um caso clínico de reimplante intencional em molar inferior com anatomia em forma de “C” associado à persistência de lesão periapical após tratamento endodôntico prévio. Paciente do sexo feminino, 37 anos, normossistêmica, compareceu à clínica odontológica com queixa de dor recorrente e desconforto mastigatório em região posterior mandibular esquerda. Relatou histórico de tratamento endodôntico e retratamento do elemento 47, sem resolução definitiva da sintomatologia. Ao exame clínico e imaginológico, observou -se lesão periapical persistente e anatomia radicular compatível com canal em forma de “C”, confirmada por tomografia computadorizada de feixe cônico. Diante da persistência da infecção e da inviabilidade de novo retratamento convencional, optou -se pela realização do reimplante intencional. O procedimento foi realizado sob anestesia local e exodontia minimamente traumática. Extraoralmente, foram executadas apicectomia, retropreparo ultrassônico obturação retrógrada com agregado trióxido mineral (MTA), com rigoroso controle do tempo extra-alveolar para preservação do ligamento periodontal. Em seguida, o elemento dentário foi reimplantado em seu alvéolo original sendo feita contenção com fio de sutura, que foi mantida por 7 dias. Nos acompanhamentos clínico e radiográfico pós-operatórios, observou-se ausência de sintomatologia dolorosa, estabilidade dentária, redução total da lesão periapical e reparo ósseo satisfatório. Conclui-se que o reimplante intencional constitui uma alternativa viável e eficaz para manutenção de dentes com anatomia complexa e falhas endodônticas persistentes, desde que realizado com planejamento adequado e criterioso controle clínico.pt_BR

dc.description.resumo_abstractIntentional replantation is a conservative therapeutic alternative indicated in cases of failed conventional endodontic treatment, enabling the maintenance of the dental element and avoiding tooth extraction. Teeth with complex root anatomy, such as molars presenting C-shaped canals, represent a clinical challenge due to the difficulties associated with instrumentation, disinfection, and adequate obturation of the root canal system. The present study aims to report a clinical case of intentional replantation in a mandibular molar with C-shaped canal anatomy associated with persistent periapical lesion after previous endodontic treatment. A 37-year-old female patient attended the dental clinic complaining of recurrent pain and masticatory discomfort in the left posterior mandibular region. The patient reported a history of endodontic treatment and retreatment of tooth 47, without definitive resolution of symptoms. Clinical and imaging examinations revealed a persistent periapical lesion and root anatomy compatible with a C-shaped canal, confirmed by cone-beam computed tomography (CBCT). Given the persistence of infection and the impracticability of further conventional retreatment, intentional replantation was chosen as the treatment approach. The procedure was performed under local anesthesia with minimally traumatic extraction. Extraorally, apicoectomy, ultrasonic retrograde cavity preparation, and retrograde filling with mineral trioxide aggregate (MTA) were carried out, with strict control of the extra-alveolar time to preserve the periodontal ligament. Subsequently, the tooth was replanted into its original socket, and stabilization was achieved with suture splinting maintained for 7 days. During clinical and radiographic postoperative follow-up, absence of painful symptoms, dental stability, complete reduction of the periapical lesion, and satisfactory bone repair were observed. It can be concluded that intentional replantation is a viable and effective alternative for maintaining teeth with complex anatomy and persistent endodontic failure, provided that appropriate planning and careful clinical control are performed.pt_BR
