Resumen : | A desinfecção eficaz do canal radicular é um componente fundamental do tratamento endodôntico bem-sucedido. No entanto podem ocorrer falhas, com remanescente de tecido infectado e inflamação perirradicular persistente. A Terapia Fotodinâmica (PDT) vem se destacando como auxiliar aos procedimentos antimicrobianos, apresentando resultados positivos como auxiliar no preparo químico-mecânico. O presente estudo objetiva apresentar um relato de caso de retratamento endodôntico não cirúrgico associado a PDT em um dente com lesão periapical persistente. Paciente J.P.S.N, 48 anos, compareceu na Clínica Escola de Odontologia do Centro Universitário Christus, queixando-se do escurecimento da coroa do dente 11. Ao realizar exame clínico e radiográfico foi observada a presença de rarefação óssea periapical na região do dente 11 e material obturador em região de câmara pulpar. A necessidade de retratamento endodôntico foi esclarecida ao paciente, para em seguida realizar o tratamento estético. Realizou-se o retratamento endodôntico não cirúrgico com os instrumentos Gates Glidden (Dentsply), lima tipo Hedstroem (Dentsply), lima tipo K (Dentsply), além da medicação intracanal à base de hidróxido de cálcio e da PDT utilizando o fotossensibilizador azul de metileno 0,01% aguardando o tempo de pré-irradiação de 5 minutos, em seguida realizado a irradiação com o laser de baixa potência DUO (MM Óptics) acoplado a uma fibra óptica (MM Óptics), utilizando os parâmetros de 660nm, 9J, 90s, 100Mw. Foram realizados 3 ciclos desse protocolo na mesma sessão. 6 meses após o tratamento ter sido concluído foi possível observar remissão gradual da lesão periapical, com ausência de sintomatologia dolorosa, indicando assim, um prognóstico favorável. |
