dc.description.abstractA periodontite é comumente afetada diretamente por fatores modificadores de biofilme, sendo importante ressaltar as condições sistêmicas, como o Diabetes Mellitus tipo 2. Objetivo do presente trabalho é relatar um caso de tratamento periodontal com protocolo full mouth disinfection associado ao uso de terapia fotodinâmica antimicrobiana (aPDT), em paciente portador de diabetes mellitus fora da meta glicêmica terapêutica. Paciente, 53 anos, sexo masculino, portador de diabetes mellitus tipo 2, fora da meta glicêmica terapêutica. Foi realizado, anamnese, exame clínico inicial, ISS, Odontograma, PSR e foi solicitado exames hematológicos. Posteriormente, foi feito o periograma para iniciar o tratamento periodontal. O protocolo de full mouth disinfection, foi feito dentro de 24 horas, dividido em 3 sessões, onde a primeira foi de raspagem supragengival de boca toda, a segunda de raspagem subgengival de toda a arcada superior, com aPDT no segundo quadrante e a terceira sessão de raspagem subgengival da arcada inferior completa, com aPDT no quarto quadrante. A programação para coleta de resultados foi agendada para sessões periódicas após o tratamento realizado, sendo com intervalo de 1 mês o primeiro retorno, seguido de retornos 4 meses, 6 meses e 12 meses após o tratamento. No periograma inicial, foi possível sangramento em todos os quadrantes, assim como ausências dentárias e bolsas acima de 4mm em pelo menos 1 sítio de cada quadrante, bem como, bolsa periodontal de 10mm. Após a realização do periograma de reavaliação com um mês de tratamento, é possível observar melhora nos parâmetros periodontais, com destaque para a profundidade de sondagem do segundo quadrante, em que antes apresentava bolsa periodontal >10mm e após o tratamento a maior bolsa do segundo quadrante foi de 5mm. O último periograma de reavaliação trouxe piora nos parâmetros examinados, destacando o sangramento à sondagem elevado, próximo ao inicial, e a profundidade de sondagem em alguns sítios acentuando-se novamente, com lesão de furca aparente. É válido ressaltar que o paciente não cooperou com o tratamento durante esse período. O presente trabalho permite analisar a terapia fotodinâmica como adjuvante ao tratamento periodontal em paciente portador de diabetes mellitus fora da meta terapêutica. Entretanto, o referido caso evidencia que o paciente tem participação fundamental no tratamento, para a eficácia do mesmo e mantimento da saúde periodontal.pt_BR

dc.description.resumo_abstractPeriodontitis is commonly directly influenced by biofilm-modifying factors, with systemic conditions such as type 2 diabetes mellitus being particularly noteworthy. The objective of this study is to report a case of periodontal treatment using a full-mouth disinfection protocol associated with antimicrobial photodynamic therapy (aPDT) in a patient with type 2 diabetes mellitus outside the therapeutic glycemic target.The patient, a 53-year-old male, was diagnosed with type 2 diabetes mellitus and presented glycemic levels outside the therapeutic target. Medical history, initial clinical examination, plaque index (PI), odontogram, and periodontal screening and recording (PSR) were performed, along with the request for hematological tests. Subsequently, a periodontal chart (periogram) was completed to initiate periodontal treatment. The full-mouth disinfection protocol was completed within 24 hours, divided into three sessions. The first session involved supragingival scaling of the entire mouth. The second session included subgingival scaling of the entire upper arch, with aPDT applied to the second quadrant. The third session focused on subgingival scaling of the entire lower arch, with aPDT applied to the fourth quadrant. Follow-up appointments were scheduled periodically after the treatment, with the first at one month, followed by assessments at 4, 6, and 12 months. The initial periogram revealed bleeding in all quadrants, missing teeth, and probing depths greater than 4 mm in at least one site per quadrant, including one site with a 10 mm periodontal pocket. After one month, the re-evaluation periogram showed improvement in periodontal parameters, particularly in the second quadrant, where a periodontal pocket initially exceeding 10 mm was reduced to a maximum depth of 5 mm post-treatment. However, the final re-evaluation periogram revealed deterioration in the examined parameters, including an increase in bleeding on probing, nearly returning to initial levels, and deepened probing depths in some sites, with furcation involvement noted. It is important to highlight that the patient did not comply with the treatment during this period. This case study allows for the analysis of photodynamic therapy as an adjunct to periodontal treatment in a patient with uncontrolled type 2 diabetes mellitus. Nonetheless, it underscores the critical role of patient cooperation in achieving effective treatment outcomes and maintaining periodontal health.pt_BR
