dc.description.abstractA osteonecrose dos maxilares é uma reação adversa grave vivenciada por alguns indivíduos devido ao uso de medicamentos comumente utilizados no tratamento de câncer e da osteoporose, como os bifosfonatos, denosumabe e agentes antiangiogênicos. Envolve a destruição progressiva do osso na mandíbula ou maxila. O crack é uma substância derivada da cocaína, e seu uso crônico pode resultar em complicações severas no tecido ósseo. O presente trabalho tem como objetivo relatar um caso clínico de osteonecrose induzida pelo uso de crack, destacando aspectos clínicos, diagnóstico e abordagem terapêutica. Paciente do sexo masculino, 58 anos, normossistêmico e sintomático, procurou atendimento na clínica de Estomatologia da Universidade Federal do Ceará. Durante a anamnese relatou estar em reabilitação ao uso de crack e que, após a exodontia dos dentes 11 e 21 para posteriormente reabilitação com prótese total, apresentou recaída ao uso da droga ilícita. Após uma semana, evoluiu com sintomatologia dolorosa, secreção purulenta e exposição óssea em região anterior de maxila. O diagnóstico foi realizado por meio de exame clínico e tomografia computadorizada, que evidenciou sequestro ósseo envolvendo região anterior de maxila. O tratamento consistiu em desbridamento cirúrgico do tecido necrótico, associado à antibioticoterapia e tratamento adjuvante com aPDT e azul de metileno. O acompanhamento pós- operatório demostrou evolução satisfatória. Conclui-se que o diagnóstico precoce e as abordagens adequadas são fundamentais para o copt_BR

dc.description.resumo_abstractOsteonecrosis of the jaws is a serious adverse reaction experienced by some individuals due to the use of drugs commonly used in the treatment of cancer and osteoporosis, such as bisphosphonates, denosumab and antiangiogenic agents. It involves the progressive destruction of bone in the jaw or maxilla. Crack cocaine is a substance derived from cocaine, and its chronic use can result in severe complications in bone tissue. The present study aims to report a clinical case of osteonecrosis induced by crack use, highlighting clinical aspects, diagnosis and therapeutic approach. A 58-year-old male patient, normossystemic and symptomatic, sought care at the Stomatology Clinic of the Federal University of Ceará. During the anamnesis, he reported being in rehabilitation for the use of crack and that, after the extraction of teeth 11 and 21 for later rehabilitation with complete prosthesis, he relapsed to the use of the illicit drug. After one week, the patient developed painful symptoms, purulent discharge, and bone exposure in the anterior region of the maxilla. The diagnosis was made through clinical examination and computed tomography, which showed bone sequestration involving the anterior region of the maxilla. The treatment consisted of surgical debridement of the necrotic tissue, associated with antibiotic therapy and adjuvant treatment with aPDT and methylene blue. Postoperative follow-up showed satisfactory evolution. It is concluded that early diagnosis and appropriate approaches are essential for the cpt_BR
