dc.description.abstractTraumas dentários são frequentes na infância e podem comprometer a integridade dos dentes decíduos e dos seus sucessores permanentes, nesses últimos, podem ocorrer sequelas associadas a distúrbios de desenvolvimento na dentição, incluindo hipoplasia de esmalte, alteração na erupção e malformações dentárias. A luxação intrusiva, ou intrusão dentária, é uma das formas mais graves de traumatismo, caracterizando-se pelo deslocamento do dente para dentro do alvéolo. Este trabalho teve como objetivo relatar a assistência odontológica imediata e mediata em um caso clínico de intrusão dentária de um dente decíduo anterior em uma paciente de 3 anos de idade, analisando suas estratégias terapêuticas e consequências para a saúde bucal infantil. A paciente foi atendida e diagnosticada com trauma por intrusão grau III do dente 61, leve mobilidade do 62 e laceração gengival associada na região acometida. Após exame clínico e exames radiológicos complementares, a primeira conduta foi a laserterapia associada ao azul de metileno (aPDT) para cicatrização da laceração gengival. Após acompanhamento de apenas dois meses do trauma, sem reerupção significativa do dente decíduo que demonstra maior segurança para o permanente, a exodontia do dente 61 foi realizada. Iniciou-se em seguida o planejamento da ortodontia preventiva com a indicação de mantenedor de espaço funcional. O primeiro mantenedor de espaço funcional foi o removível com acréscimo de dente unitário, mas que por ser removível, o seu uso diário não foi efetivo. Um novo aparelho mantenedor fixo foi planejado através do fluxo digital. Após instalado, o dispositivo não teve uma retenção adequada efetiva, saiu de posição, causando uma descontinuação no seu uso. Neste momento foi necessário, por fim, um conserto do dispositivo substituindo suas bandas por um tamanho 0,5 menor para melhorar a sua retenção através da moldagem convencional com as novas bandas selecionadas em boca. Foi mantido acompanhamento periódico da paciente em todos os períodos com adequação do meio quando necessário. O estudo reforça a relevância do diagnóstico precoce, acompanhamento odontológico contínuo com aplicação de técnicas de manejo e replanejamento necessário e adequado ao traumatismo dentário na primeira infância, visando minimizar complicações funcionais e estéticas futuras. O acompanhamento especializado foi essencial para diagnóstico, abordagem do trauma dentário e replanejamento adequando a situação da paciente na primeira infância, garantindo um prognóstico favorável.pt_BR

dc.description.resumo_abstractDental trauma is frequent in childhood and may compromise the integrity of primary teeth and their permanent successors; in the latter, sequelae associated with developmental disturbances in the dentition may occur, including enamel hypoplasia, altered eruption, and dental malformations. Intrusive luxation, or dental intrusion, is one of the most severe forms of trauma, characterized by the displacement of the tooth into the alveolar socket. This study aimed to report the immediate and mediate dental care in a clinical case of dental intrusion of an anterior primary tooth in a 3-year-old patient, analyzing its therapeutic strategies and consequences for children's oral health. The patient was treated and diagnosed with grade III intrusion trauma of tooth 61, slight mobility of tooth 62, and associated gingival laceration in the affected region. After clinical examination and complementary radiological exams, the first approach was laser therapy associated with methylene blue (aPDT) for healing of the gingival laceration. After only two months of follow-up since the trauma, with no significant re-eruption of the primary tooth — which would provide greater safety for the permanent successor — extraction of tooth 61 was performed. Preventive orthodontic planning was then initiated with the indication of a functional space maintainer. The first functional space maintainer was removable with the addition of a single artificial tooth; however, since it was removable, its daily use was not effective. A new fixed space maintainer was planned using a digital workflow. After installation, the device did not achieve adequate retention and became dislodged, causing discontinuation of its use. It was then necessary to repair the appliance by replacing its bands with a size 0.5 smaller to improve retention, using conventional impressions with the newly selected bands placed intraorally. Periodic follow-up of the patient was maintained throughout all stages, with environmental adjustments whenever necessary. The study reinforces the relevance of early diagnosis, continuous dental follow-up with the application of behavior management techniques, and appropriate replanning of dental trauma in early childhood, aiming to minimize future functional and aesthetic complications. Specialized follow-up was essential for diagnosis, management of dental trauma, and adequate replanning adapted to the patient's situation in early childhood, ensuring a favorable prognosis.pt_BR
