Abstract: Infiltrantes de resina são biomateriais capazes de tratar a cárie de esmalte em estágios iniciais, sendo hábil em penetrar nas estruturas dos capilares do corpo da lesão. Apresenta índice de refração semelhante ao esmalte saudável podendo equilibrar a diferença na refração da luz e aproximar a aparência da lesão de mancha branca infiltrada à do esmalte saudável adjacente. Contudo alta rugosidade de superfície é observada após sua aplicação, podendo aumentar o acúmulo de biofilme dental e assim comprometer a longevidade. Um polimento à superfície do material infiltrante (Icon®) pode auxiliar no controle da adesão do biofilme à superfície do material, evitando a reincidência de lesões cariosas ou a progressão das mesmas. Deste modo, o objetivo desse estudo foi avaliar o efeito de diferentes sistemas de polimentos ao infiltrante em relação à rugosidade e adesão microbiana. Para isso, experimento in vitro seguido por um modelo in situ para formação de biofilme foi realizado. Na fase in vitro, lesões cariosas artificiais foram produzidas em espécimes de esmalte bovino (4x4x2mm) através do protocolo de ciclagem desmineralização-remineralização. Posteriormente os espécimes foram tratados com o infiltrante e aleatoriamente divididos nos seguintes grupos (n=15): sem polimento (SP), polimento com discos flexíveis de óxido de alumínio (PDOA), pontas siliconadas de carbeto de silício (PSCS), tiras de poliéster de carbeto de silício (PCS) e escova de carbeto de silício (PECS). Nesta etapa, os espécimes foram analisados quanto à rugosidade de superfície. No estudo in situ, 15 voluntários utilizaram dispositivos intraorais por 14 dias, induzindo o acúmulo de biofilme por meio de gotejamento de solução de sacarose 8x/dia. O biofilme formado foi removido e analisado com relacão à contagem de Streptococcus mutans, Lactobacillus ssp e Microorganismos totais. Após coleta de todos os dados, inicialmente o teste Shapiro Wilks foi realizado, os dados de rugosidade foram analisados por teste Kruskal Wallis, mostrando que há efeito do tipo de polimento sobre o padrão de rugosidade (p=0,001). A comparação entre os pares mostraram diferença estatística entre controle e PDOA (p<0,001) e entre controle e PCS (p=0,040). Na análise microbiológica, foi realizado ANOVA seguido de Tukey. Na contagem de Streptococcus mutans e Lactobacillus ssp, não houve diferença estatística (p= 0,35 e p= 0,058, respectivamente). Contudo, nos Microorganismos totais houve diferença estatística (p=0,044). PDOA apresentou menor crescimento de Microorganismos totais em relação ao grupo que foi utilizado PECS (p= 0,037). Os padrões de polimento com PDOA e PCS de resina apresentaram menor rugosidade de superfície, contudo a maior lisura proporcionada por esses materiais não reduziu a adesão de Streptococcus mutans, Lactobacillus ssp em comparação ao controle e aos outros polimentos sobre a superfície do infiltrante de resina. Assim o infiltrante resinoso apresentou rugosidade de superfície diminuída dependendo do sistema de polimento utilizado, contudo a menor rugosidade alcançada pelos sistemas PDOA e PCS não foi suficiente para ter uma redução significativa da adesão microbiana.
Description: Resin infiltrates are biomaterials capable of treating enamel caries in the early stages, being able to penetrate the capillary structures of the lesion body. It presents a refractive index similar to healthy enamel, which can balance the difference in light refraction and approximate the appearance of the white spot lesion infiltrated to that of the adjacent healthy enamel. However high surface roughness is observed after its application, which may increase the accumulation of dental biofilm and thus compromise longevity. A surface polishing of the infiltrating material (Icon®) may aid in the control of the biofilm adhesion on the surface of the material, avoiding the recurrence or progression of carious lesions. Thus, the objective of this study was to evaluate the effect of different polishing systems on the infiltrating Icon® in relation to roughness and microbial adhesion. For this, an in vitro experiment followed by an in situ model for biofilm formation was performed. In the in vitro phase, artificial carious lesions were produced in bovine enamel specimens (4x4x2mm) through the demineralizationremineralization cycling protocol. Subsequently the specimens were treated with the infiltrating agent and randomly divided into the following groups (n = 15): non-polishing (SP), polishing with aluminum oxide flexible discs (PAOF), silicon carbide silicone tips (SCST), silicon carbide polyester strips (SCP) and silicon carbide brush (SCB). At this stage, the specimens were analyzed for surface roughness. In the in situ study, 15 volunteers used intraoral devices for 14 days, inducing the biofilm accumulation by dripping 8x / day sucrose solution. The formed biofilm was removed and analyzed for the count of Streptococcus mutans, Lactobacillus ssp and Total microorganisms. After data collection, initially the Shapiro Wilks test was performed, the roughness data were analyzed by Kruskal Wallis test, showing that there is an effect of the polishing type on the roughness pattern (p = 0.001). The comparison between the pairs showed a statistical difference between control and PAOF (p <0.001) and between control and SCP (p = 0.040). In the microbiological analysis, ANOVA followed by Tukey was performed. In the counts of Streptococcus mutans and Lactobacillus ssp, there was no statistical difference (p = 0.35 and p = 0.058, respectively). However, in the total Microorganisms there was statistical difference (p = 0.044). PAOF presented lower growth of total microorganisms in relation to the group that used SCB (p = 0.037). Polishing patterns with PAOF and SCP resin showed less surface roughness, however, the greater smoothness provided by these materials did not reduce the adhesion of Streptococcus mutans, Lactobacillus ssp compared to control and to the other polishes on the surface of the resin infiltrate. Thus, the resinous infiltrate presented reduced surface roughness depending on the polishing system used, however, the smaller roughness achieved by the PAOF and SCP systems was not enough to reduce the microbial adhesion.
